How it Works
Please enter as much information as you can, then click the "Submit Report Now" button found at the end of the form.
When submitted you will be given confirmation of receipt of the report along with an Incident Number. This number will allow you to check the status of the investigation, including if a reward is being paid or if additional information is required.
